Teen Artist Intake Form

Complete this form to request exhibition of your artwork at the Laramie County Library.

You will need the following for this form:

1. A photo of your artwork (single piece) or a photo of yourself (with your face visible).
2. The date(s) your art teacher is available to help setup artwork and attend your reception.
3. An artist’s statement: (50 words or less) A written description of your work (which might include mediums used, themes, subjects etc.) to help viewers better understand your art.
4. Your teacher’s name and email.
Reminder: Your art will be displayed in the library so please select art selections appropriate for a public space. You will be responsible for transporting your art to and from the library as well as promptly taking down work in time to allow for the next student exhibition.
